Residence Permit: Valid residence permit if applicant is a foreigner staying in Ghana.
Photograph: One (1) American size passport picture.
Reservations: Flight reservation and/or Hotel reservation.
2. MEDICAL & HOSPITAL DOCUMENTS
Hospital Invitation: Hospital invitation letter from Turkey (Must be written in Turkish).
Referral Letter: Letter of referral from a Government Hospital in Ghana (Compulsory).
Health Türkiye: Applicants must notify the hospital in Turkey to register their application with the Health Türkiye Portal.
Payment: Proof of transfer of funds (if any).
3. PROFESSIONAL & FINANCIAL DOCUMENTS
Company Letter: Introduction letter from your company on letterheaded paper showing employment date, salary, and promotions (if applicable).
Registration: Certificate of company registration and tax clearance certificate (Originals and photocopies).
Income: 3 months payslips (if applicable).
Bank Statements: 3 months bank statement of sponsor and applicant showing enough funds (Signed and stamped by the bank).
Sponsorship: Sponsorship letter if you are not sponsoring your trip.
IMPORTANT REQUIREMENTS:
Travel Insurance: Compulsory to be obtained from the visa center.
Spouse/Family: Include an original marriage certificate if traveling with a spouse, and an original birth certificate if traveling with a parent or sibling.
All bank statements should reflect the most recent date.
